How It Works

Receiving support doesn’t have to feel complicated or overwhelming. At Rise & Restore Home Wellness, we follow a simple, thoughtful process designed to respect your privacy, pace and comfort. From the first conversation to ongoing support, our approach is centered on clarity, dignity and steady presence- so help feels supportive, not intrusive

Step One : Reach Out

Getting started is simple and confidential.

Start by sending us a message through our contact form (or by phone/email). You don’t need to have everything figured out - just tell us what’s feeling heavy right now and what kind of help would make life feel more manageable . We respond thoughtfully and as promptly as possible.

Step Two: A Quiet Conversation

We’ll schedule a short, private intake call to understand:

  • What support you need (and what you don’t need)

  • Your schedule and routines

  • Your preferences (personality, pace, boundaries, cultural comfort, home expectations)

  • Safety Notes and household details (pets, stairs, mobility, etc. )

This helps us match you thoughtfully and set a plan that feels steady- not overwhelming.

Together, we create a supportive care plan that reflects your current season of life, this may include:

  • Household support

  • Child supervision (with client present)

  • Companionship and emotional presence

  • Meal preparation and light daily living assistance

  • Personal support with activities of daily living (as appropriate)

Our focus is always on preserving dignity, routine, and peace in your home.

If this feels like the kind of support you’ve been needing, you don’t have to wait until everything feels urgent.

Step 3: Your Support Plan & Schedule

After intake, we create a simple support plan that outlines:

  • your goals for support

  • What tasks are included (and excluded)

  • Frequency and length of visits

  • any special note for consistency and comfort.

You’ll receive clear confirmation of your schedule and what to expect before services begin.

Step Four: Matching You With the Right PSW

we don’t “ assign whoever is available”. We match based on:

  • Your needs and comfort level

  • personality fit

  • availability and consistency

  • experience with mental health, dementia/ cognitive support or palliative support (as needed)

When possible, we keep your care consistent with the same worker(s)

Step Five: Support Begins

Your PSW arrives at the scheduled time and provides support based on your plan.

Support may include:

  • Companionship and emotional presence

  • gentle routine support (meals, laundry, light housekeeping, errands)

  • Cognitive support (memory cues, calm structure, safety reminders)

  • Personal care support (as appropriate and agreed upon

  • Family - present support (extra hands while you’re still home)

We focus on dignity, calm, and steadiness - so your home feels supported, not disrupted.

Step Six: Ongoing Care & Check-ins

We stay connected.

As your needs evolve, we adjust the care plan accordingly. Our goal is not dependency - Its stability, relief and restored balance.

Your always in control of your care.

Step Seven: Ongoing Communication & Adjustments

Life changes, so support can change too. You can request:

  • Schedule updates

  • Additional hours

  • a different support focus

  • a new worker match if needed

We also welcome feedback at any time - quietly and respectfully.
